1. Successful Treatment of Genital Warts with Ingenol Mebutate Monitored with Optical Coherence Tomography and Reflectance Confocal Microscopy

  Ingenol mebutate (IM) is approved for the treatment of actinic keratosis and induces cell death in precancerous lesions. The efficacy of IM in the treatment of genital warts was investigated in a therapy-refractory patient. The 74-year-old male was treated with IM gel for three consecutive days. Treatment course and efficacy were evaluated by clinical inspection and non-invasive diagnostics namely optical coherence tomography (OCT) and reflectance confocal microscopy (RCM). Within 24 to 48 hours IM induced a strong local inflammatory reaction. One week later a complete response was observed.   The conversion of hyporeflective basal cell carcinoma nodules into hyperreflective ovoid structures was remarked with optical coherence tomography (OCT) as a sign of tumor regression under photodynamic therapy (PDT). OCT presented a higher accuracy in identifying residual and non-residual lesions after PDT treatment compared to clinical inspection alone. OCT might be used in the future as a standard procedure for detecting non-melanoma skin cancer sparing patients invasive procedures.

  High-definition optical coherence tomography and the corresponding vertically sectioned histology of a compound naevus. High-definition optical coherence tomography (HD-OCT) scanners have recently been developed. We assessed micromorphological HD-OCT correlates of benign naevi (BN) and malignant melanoma (MM). 28 BN and 20 MM were studied using HD-OCT and histology. Epidermal honeycomb/cobblestone pattern, regular junctional cell nests, and edged papillae are more often observed in BN, whereas fusion of rete ridges, pagetoid cells and junctional and/or dermal nests with atypical cells are more frequently seen in MM. The evaluation of tumor regression has been performed using clinical photography and radiographic scans.
